CSH9 - Simple DC coupled power amp with pre-amp, volume control and DC servo

I've seen a few people asking for a simple design, that has enough gain to run directly from a portable MP3 player. This one conveniently has a volume control, uses no electrolytic caps in the "signal path", and has a DC servo to keep the output nulled in the face of bias currents in the opamps and LTP. Its designed to be simple and easy to build.

Please let me know if you spot any errors or areas for improvement.
